Documentation: Update index.md and move files
* Add more subdirectories and index.mds.
* Move "getting started" and "lessons" into sub-directories.
* Move "NativeRaminit" into northbridge/intel/sandybridge folder.
* Move "MultiProcessorInit" into soc/intel/icelake folder.
