Previously, in case of error in fz_jpg_mem_init(),
jpeg_finish_decompress()/jpeg_destroy_decompress()
would be called before jpeg_create_decompress() had
initlized all state. This sometimes led to segfaults
or accessing uninitialized data.

Getting a backtrace out with missing functions makes the
backtrace much less useful. Some backtrace routines (such
as that used by Memento on Android) are incapable of
resolving static functions.
We therefore provide 2 scripts (scripts/destatic.sh and
scripts/restatic.sh) that respectively remove and replace
the 'static' from function definitions.
The scripts do not affect "static inline" or "static const"
definitions, and they are are restricted to working in the
source directory (excluding source/tools), thirdparty/mujs
and the platform/{java,android} directories.
The transformed source should NOT be checked in.
To avoid problems with clashing symbols, some functions are
renamed or tweaked slightly in this patch.

This silences the many warnings we get when building for x64
in windows.
This does not address any of the warnings we get in thirdparty
libraries - in particular harfbuzz. These look (at a quick
glance) harmless though.

We compile with -Wno-unused-parameters instead.

Purge several embedded contexts:
Remove embedded context in fz_output.
Remove embedded context in fz_stream.
Remove embedded context in fz_device.
Remove fz_rebind_stream (since it is no longer necessary).
Remove embedded context in svg_device.
Remove embedded context in XML parser.
Add ctx argument to fz_document functions.
Remove embedded context in fz_document.
Remove embedded context in pdf_document.
Remove embedded context in pdf_obj.
Make fz_page independent of fz_document in the interface.
We shouldn't need to pass the document to all functions handling a page.
If a page is tied to the source document, it's redundant; otherwise it's
just pointless.
Fix reference counting oddity in fz_new_image_from_pixmap.

Rather than decoding entire image only to give up after we find the
very first pixel is color, add code so that the test-device can
treat the image as a stream. This means that (for most image types
at least) we can bale out without decoding everything.
This reduces the runtime of 3001Pages.pdf from 14 minutes to 18 seconds.

This adds a custom memory management layer between libjpeg and the calling
app - in such a way that the code can be shared between mupdf and
Ghostscript/PDL.

Currently fz_streams have a 4K buffer within their header. The call
to read from a stream fills this buffer, resulting in more data being
pulled from any underlying stream than we might like. This causes
problems with the forthcoming 'leech' filter.
Here we simplify the fields available in the public stream header.
No specific buffer is given; simply the read and write pointers.
The underlying 'read' function is replaced by a 'next' function
that makes the next block of data available and returns the first
character of it (or EOF).
A caller to the 'next' function should supply the maximum number of
bytes that it knows it will need (possibly not now, but eventually).
This enables the underlying stream to efficiently decode just enough.
The underlying stream is free to return fewer, or a greater number
if it wants to.
The exact size of the 'block' of data returned will depend on the
filter in use and (possibly) the data therein.
Callers can get the currently available amount of data by calling
fz_available (but again should pass the maximum amount of data they know
they will need). The only time this will ever return 0 is if we have
hit EOF.

Using JDCT_FASTEST as rendering method can produce visible artifacts
(e.g. in 1960_-_DCT_image_wrongly_decoded_regression_from_1.2_.pdf).

This is required e.g. for 2314 - jpeg tables in tiff.xps.
This folds fz_open_resized_dct back into fz_open_dct instead of adding
further variations for calls with and without the jpegtables argument.

We are testing this using a new -p flag to mupdf that sets a bitrate at
which data will appear to arrive progressively as time goes on. For
example:
mupdf -p 102400 pdf_reference17.pdf
