For methods that are only exposed for testing. Once they are static,
there is no need for dummy CPDF_StreamContentParser constructor calls in
the unit tests. Adjust the CPDF_StreamContentParser constructor now that
one of the parameters can no longer be a nullptr.

This CL separates pageint.h and the supporting cpp files into indivudal
class files.

- The code in fpdf_page_parser is only called by CPDF_StreamContentParser, so moved there.
- Split fpdf_page_parser_old into its two classes
- Renamed the corresponding unittests accordingly.
- Moved PDF_ReplaceAbbr to namespace
- Fixed few nits
- Added TODO because CPDF_StreamParser has a lot of code similar to CPDF_SyntaxParser
