PDF decoders, AKA filters, can be chained together. There can be
an arbitrary number of decoding / decompressing filters in the pipeline,
but there should be at most 1 image decoder, and the image decoder
should only be at the end of the chain.

Given a span of size N, memcpy(dest, &span[N], 0) ought to be a no-op,
but since we compute span[N] before checking for zero length, we hit
an assert. The correct idiom should be to create a sub-span, which
allows specifying N, but only when the size is 0.

In text like document title 0x001B is used as a marker for the
beginning/end of a language metadata section. Currently PDFium does
nothing with this data, but when returning the 'decoded' text it needs
to be stripped out.
The existing code assumed that the two bytes following a marker would
be the data to be removed and did nothing to track if it was in/out of
one of these regions. This led to a situation where it would always
strip the two bytes following the region, since it assumed the end
marker was the beginning of a new region.
This CL corrects the detection and handling of these regions, and adds
a regression test for the reported bug.

Picks up some enhancements from base/span.h. In turn, also adds
the size_bytes() helper. Differs from base version in that it
works around C++14 enable_if_t<>, and avoids the dynamic_extent
template specialization tricks.
Use it in a few places where appropriate.

Instead of one general RemoveAt() method, split it into:
- RemoveAt(index)
- Truncate(nNewSize)
- Clear()
Update callers, which are now easier to understand.
Update existing unit tests and add new tests.
