Reason for revert: (void) idiom is prevalent through out C++ code.

Applies std::remove_ptr to the public API types so that we can
deduce a correct unique ptr type no matter how that API might
change away from void* usage.
Creates shorter names for std::unique_ptr<std::remove_pointer<>, ...>

This CL splits some helper code out of pdfium_test into individual
files. This makes it a bit clearer what each chunk of code is doing and
makes pdfium_test easier to read.
