This CL removes the CXFA_FFDocView parameter from CXFA_WidgetAcc and
passes in the document to the methods which require a document.

We never use ::GetAccess to get the value in general, just to see if
it's set to Open or not. This CL makes the usage clearer.

This CL converts CXFA_BindData::GetPicture and
CXFA_WidgetData::GetPictureContent to return the WideString instead of
taking an out param.
The original bool result was never checked.

This CL splits the node template out of xfa_utils into its own file. The
XFA_ByteStringToDouble method was moved to the only calling file.

XFA bits passed into the PDFFontMgr are just used to get the
CPDF_Document and the CFGAS_FontMgr. This CL changes the code to pass
those two things into the constructor. Then, the fxfa/cxfa_pdffontmgr.*
code has been moved to fgas/cfgas_pdffontmgr.* to show it doesn't depend
on any of the XFA classes.

Automated using git grep & sed.
Replace StringC classes with StringView classes.
Remove the CFX_ prefix and put string classes in fxcrt namespace.
Change AsStringC() to AsStringView().
Rename tests from TEST(fxcrt, *String*Foo) to TEST(*String*,
Foo).
Couple of tests needed to have their names regularlized.

This CL rebuilds the text edit engine in a simpler fashion. Instead of
depending on multiple pages, paragraphs and buffer fields there is a
single text edit engine which contains a gap buffer.
This makes the code easier to understand and follow.

There currently exists a weird split where some files exist in xfa/fxfa
and some files exist in xfa/fxfa/app. This CL removes the app/ folder
and moves all files up to the parent directory.
